Functional Direct Primary Care FAQ

Functional Direct Primary Care is a model of healthcare delivery that allows doctors and patients to work together to achieve optimal health without many of the barriers put in place by health insurance companies.
By paying a monthly or annual membership fee, you will have access to schedule primary care appointments, follow-up visits, same-day or next-day acute care visits, common in-office testing and procedures, and access to an in-house nutritionist and strength coach along with Dr. Strandberg and the Rise Health care team.

The insurance system requires high patient volume and short visits to generate sufficient billing revenue. That model does not work with the functional medicine / whole-person approach, which requires longer appointments, comprehensive history-taking, detailed lab interpretation, and individualized treatment planning. Functional Direct Primary Care allows Dr. Strandberg and the Rise Health care team to spend enough time with each patient, and provide the depth of care that reflects her training as a Doctor of Osteopathic Medicine and accreditation through the Institute of Functional Medicine.
Since opening Rise Health in July of 2025, it has been an honor to provide top quality and personalized healthcare to our patients. In order to continue providing care with the same excellence and flexibility that the practice was founded on, including extended visits, prompt appointments and direct communication, along with adding nutrition and strength support, Rise Health is transitioning to Functional Direct Primary Care beginning on January 1, 2027.

Functional Direct Primary Care at Rise Health may be right for you if you want:

This is not a standard primary care practice — it is a functional medicine practice led by an IFM-certified physician with advanced specialty training in hormones, gut health and root-cause health and longevity. Visits are longer and more comprehensive. For comparison, a single consultation with a functional medicine specialist at a functional medicine clinic typically costs $300–$600 out-of-pocket. At $250/month, you receive unlimited visits, direct physician & care team access - including our in-house nutritionist and strength coach support. Additionally, wholesale labs, and discounts on InMode aesthetic women’s wellness treatments, Biote hormone services and IV nutrition are included. For many patients, this is equal to or less than what they currently pay in primary care copays, deductibles, and functional medicine consultations combined.

No. It is recommended you keep your health insurance for hospitalizations, specialist visits, emergency care, and any labs or prescriptions processed through insurance. Your Functional Direct Primary Care membership covers your primary care relationship — the two work together.
You may consider transitioning to a higher deductible insurance plan or a health sharing community plan to cover hospitalizations, emergency care, specialist visits and imaging.

In the state of Wyoming, as of January 1, 2026, patients enrolled in High-Deductible Health Plans (HDHPs) can use HSA funds to pay Direct Primary Care membership fees — up to $150/month for individuals and $300/month for families. Your Functional Direct Primary Care $250 membership fee falls within the family HSA limit and is partially offset by the individual limit. Please confirm with your HSA administrator.

Functional Direct Primary Care members receive a “Skin & Wellness Bank” that includes a monthly $100 credit. Members may accumulate the monthly “Bank” for up to 6 months ($600) which can be used towards InMode regenerative skin and women’s health packages.

InMode platform treatments for skin health and vaginal health include: Morpheus8 (face and body), Lumecca, Diolaze, Vaculaze, FormaV, VTone, and Morpheus8V.

BioTe uses small bioidentical hormone pellets inserted just under the skin (typically in the hip) in a brief in-office procedure. The pellets release consistent hormone levels over 3–6 months. This is an extension of the physician's hormone specialty training, and may be suited for patients managing menopause, andropause, and hormone-related symptoms. The pellets and insertion fee are billed separately with a 20% member discount and labs are available at negotiated cash rates or can be billed through insurance.
*If you are not a member, Biote pellets are available at full price.

Functional Direct Primary Care membership fees are not covered by Medicaid or Medicare. All current patients of Rise Health are welcomed to continue as patients when this transition occurs. You can still use your Medicare benefits for labs,specialist referrals, preventive care procedures, medications, and any hospital services you need.
Patients with Medicaid or Medicare who wish to become a member may pay the monthly fee directly.
If this is not feasible, and you do not plan to remain a patient of Rise Health once the practice is no longer participating in Medicare billing, please be aware that area insurance-based practices may be booking out new patient appointments several months in the future and contacting them sooner rather than later will allow for a smooth transition of care.
Please ensure that you have identified a new primary care office to assume your care as of January 1, 2027, including medication refills, referrals, and other services you may require. Records will be transferred to your new primary care office free of charge with a signed medical records release, which can be requested by the receiving office.

As a member of Rise Health Functional Direct Primary Care, the Body Recomposition Program is covered in your membership and you will simply pay the cost of the medication directly to the pharmacy. The prescription cost ranges from $150 to $450 per month depending on your medication and dosing. We are happy to give you more specific costs to better estimate your individualized plan.
*If you are not a member of Rise Health Functional Direct Primary Care, the Body Recomposition Program will remain the same and the cost for that program will still include physician supervision, quarterly InBody check-ins and the individualized medication.

Your treatment plan and prescriptions are covered with your monthly membership .
If you choose not to become a member, your new clinician will need to prescribe these medications.

No is the simple answer. However we will have transparent, negotiated cash prices available for labs. We will also have a list of cash prices for imaging for local facilities. You may also bill these items through your insurance.

We do understand that this model of care is not for everyone.
If you are a current patient, please ensure that you have identified a new primary care office to assume your care as of January 1, 2027 - including medication refills, referrals, and other services you may require. Records will be transferred to your new primary care office free of charge with a signed medical records release, which can be requested by the receiving office. Contact us before December 1, 2026 if you need assistance to ensure we have time to support your transition.

Enrollment is limited to ensure optimal patient care. The open enrollment window for founding members will begin September 1, 2026 and close November 30, 2026.
