Rise Medical Practice is transitioning to a Functional Direct Care membership model on January 1, 2027 - and enrollment is open now.

FDC is a simple idea: instead of billing insurance for every visit, you pay a monthly (or yearly) membership fee that covers all of your primary care needs. No copays. No referral hoops. No rushing through 10 minute appointments. Just a real relationship with your doctor and care team.

Erica Strandberg, DO, FMCP-M is trained in both Family Medicine and Functional Medicine because she believes in whole-person care, getting to the root-cause of health problems — not just managing symptoms.

Functional Direct Care gives Dr. Strandberg and the care team time for longer appointments, comprehensive history-taking, detailed lab interpretation and individualized treatment planning with freedom to practice the way we believe medicine should be practiced: thorough, personal, and proactive.

Comprehensive visits with Dr. Strandberg and the care team

Same-day and next-day acute-care appointments, including telehealth as needed
Direct access to your care team via portal, phone and secure messaging
Quarterly visits with in-house Nutritionist and Strength Coach
Coordination of specialist referrals
